How do Revivify Health consultations work?

All you need to do is click the ‘Request Appointment’ button below to schedule your first consultation. You will need to complete intake forms prior to your scheduled appointment with NP Kendall Schoenekase. You will also receive an email with a link to your virtual appointment. After your appointment, any medications ordered will be shipped to your doorstep.

Is this confidential?

Yes. Your medical data and order history can only be viewed by our medical team and prescribers. You can view our privacy policy for more information.

How often will I receive a shipment?

Your shipment will arrive within 3-7 days after a consultation. Be sure to book your follow up appointments at least a week before running running out of medication.

How much do medical weight loss services cost?

Initial consultations cost $150 (25% off this for referring a friend). Monthly follow up visits are $75 (50% off this for referring a friend). Prescription prices vary by the type of medication and pharmacy cost. You can expect to pay around $400 for a 2.5 MONTH SUPPLY of generic Ozempic, including all supplies and shipping! Larger vials are also available for purchase, which are even more cost effective.

How much do longevity / NAD+ services cost?

Consultations: $75, ONLY required once annually

NAD+ 500mg Vial: $225 ($45 per week, 5 week supply)

NAD+ 1000mg Vial: $295 ($29.5 per week, 10 week supply)

Follow up visits only required once every 12 months, otherwise you can request refills as often as you like :)

Who is eligible for longevity treatment?
